My friend, Jenn gave me this nearly three years ago...I think. It's to record all the pertinent info when we finally get "the call". I greatly appreciate her putting this together as I am sure that when "the call" does finally come, I will probably be in such shock and disbelief we won't remember what to ask. Thanks so much Jenn!
